The Children's Health Homes of Upstate New York, LLC (CHUNNY) operates a Health Home for Children using a model based upon the belief that every family has strengths and are the natural and most effective drivers of their care.

Provides case/care management to children and youth that are experiencing significant behavioral, medical, or social risk factors.

The Center for Youth provides free, voluntary, and confidential counseling, emergency shelter, transitional and permanent housing, crisis intervention services including crisis nursery services for youth and their families.

Helps kids with different problems through case managers. They can help with:Getting ready for a job.Doing well in school.Finding a place to stay.Getting food.Getting clothes.Getting transportation.Dealing with the law.Having fun and learning new things.
