• Provides resources and education by community health nurses to pregnant women, post partum women and pediatric patients that offer:
• Provides screening for lead poison.
• Offers the Pediatric Home Visit Program for children with acute/chronic health needs.
• Offers the Early Intervention Program for children with disabilities or delayed development.
• Offers the Infant Child Health Assessment Program/CHAP that provides screenings for physical, social and language development.
- Medical monitoring.
- Health education.
- Pre- and post-natal care.
- Case management for high risk pregnancies.
• Provides screening for lead poison.
• Offers the Pediatric Home Visit Program for children with acute/chronic health needs.
• Offers the Early Intervention Program for children with disabilities or delayed development.
• Offers the Infant Child Health Assessment Program/CHAP that provides screenings for physical, social and language development.
Eligibility: Immunization Clinic: Ages 2 months to adult. Early Intervention Program: Ages birth to 3 years. Other programs: Call for information. Income guidelines may apply for some programs and services.
