• Offers Community Health Services for individuals who are currently incarcerated.
- Outreach and education to prevent the spread of HIV, Hepatitis C, and STI's, including peer training.
- Free testing for HIV, STD's, and Hepatitis C.
- Assists individuals living with HIV & Hepatitis C in developing a care plan for when they transition back to community.

Eligibility: Must be currently incarcerated.
Application Process: Call for information. DOCCS referrals to program.
Required Documentation: DOCCS referral information.
Fees: No fee for service.
Coverage Note: Cayuga, Herkimer, Jefferson, Lewis, Madison, Oneida, Onondaga, Oswego, and St. Lawrence Counties.
Cultural Competency: Cultural competency, diversity, equity, and inclusion training for staff
Languages: English- Staff provides service in language offered, Language line
