Youth, ages 12-21 years. Preference given to those previously in a treatment program, under foster care, or in custody of the Commissioner of Social Services.

Very low to moderate income families, seniors, veterans and persons with disabilities - home owners. Income guidelines apply.

10-21 years of age.

Must be 18 years and older and show proof of brain injury or chronic health​/medical issues.

In order to be eligible for Health Home services, the individual must: Be 21 years or older Enrolled in Medicaid Live within the following New York counties: Monroe, Ontario, Livingston, Wayne, Seneca and Yates And have one or more of the following conditions: Significant mental illness or serious emotional disturbance HIV​/AIDS Two or more chronic health conditions (i.e. asthma, diabetes, heart disease, mental health condition, substance use disorder, etc.)
